Helpful Reminders to Speed up the Issue Process

To avoid customer delays, please be sure you submit a completed Proof of Good Health Acknowledgement Form with any enrollment applications requiring medical underwriting or the applications will be pended by ASI. For your convenience, these forms are posted on the WSP website under the Applications and Enrollment Forms tab. Please call WSP if you have any questions. In addition, when submitting employee enrollment applications, please make sure the form numbers are visible and clear at the bottom of each application.
