Community Health Survey

Help determine the most important actions that need to be taken to address the identified health issues for our community. Please participate in a statewide Community Health Survey being conducted by the local health councils of Florida.  As you may know, they are the designated health planning agencies for Florida, as established under F.S. 408.033. The purpose of the local health councils is to plan for local health needs and advise the state on healthcare issues and resource allocations. This research effort by the health councils is sponsored by the Florida Department of Health and the Agency for Health Care Administration. 
 
The key informant survey is designed to obtain valuable information about your views on health issues affecting your community. It is particularly focused on health access and barriers to care, as well as what you envision to be the most important actions that need to be taken to address the identified health issues for your community.
 
To complete the survey, visit www.hcecf.org/survey.html. The deadline for completion is October 15, 2002. The survey consists of 10 questions and should take less than 5 minutes to complete.
 
A summary report of the survey results will be provided to all participants. These findings will be utilized to craft health policy recommendations and strategies for each of the health-planning districts throughout the state.
