In order to receive the pre-registered price of $18.00 please mail this registration form to the Chapter office by Monday, March 27, 2006. Registrations made by phone after Monday, March 27, at 5:00 p.m. will be $20.00.

WALK-INS WILL BE PERMITTED UPON AVAILABILITY ONLY. The room is reserved based on the head count on Tuesday prior to a Friday luncheon. We cannot always accomodate all walk-ins. Please help us serve you better by registering and paying early.
